CalAIM an Innovative Approach to Healthcare
CalAIM (California Advancing and Innovating Medi-Cal) is a comprehensive initiative led by the California Department of Health Care Services (DHCS) to transform and strengthen Medi-Cal (California's Medicaid program). The program's primary purpose is to provide more whole-person care through an integrated approach that addresses medical and social needs. This includes integrating physical health, behavioral health, and social services into a coordinated system of care.
CalAIM aims to improve health outcomes by addressing Social Determinants of Health (SDOH), managing care for high-need, high-cost beneficiaries, and making Medi-Cal services more standardized and coordinated statewide. The initiative transforms the delivery system by implementing Enhanced Care Management (ECM) as a new statewide benefit, providing Community Supports to address social needs, and streamlining programs and services into one coordinated system.
CalAIM represents a significant reform of California's Medicaid system, aiming to make it more equitable, coordinated, and focused on preventing rather than just treating health conditions.

Housing transition services provide comprehensive support to members in obtaining and maintaining stable housing by conducting tenant screenings and housing assessments, developing individualized support plans, searching for and securing housing, assisting with benefits advocacy, identifying rental subsidies and financial resources, supporting reasonable accommodation requests, and engaging with landlords. These services aim to address housing barriers, establish measurable goals, and help members navigate the complex process of finding and maintaining appropriate housing through personalized, holistic support.

Housing Deposits assist with identifying, coordinating, securing, or funding one-time services and modifications necessary to enable a person to establish a basic household that do not constitute room and board, such as set-up fees, first month’s rent and utilities, and more.

This service provides tenancy and sustaining services, with a goal of maintaining safe and stable tenancy once housing is secured. Ongoing support to help individuals maintain their housing stability through continued case management and intervention.

Short-Term Post-Hospitalization Housing provides Members who do not have a residence and who have high medical or behavioral health needs with the opportunity to continue their medical/psychiatric/substance use disorder recovery immediately after exiting an inpatient hospital, residential substance use disorder treatment or recovery facility, residential mental health treatment facility, correctional facility, nursing facility, or recuperative care.

Recuperative Care, also referred to as medical respite care, is short-term residential care for individuals who no longer require hospitalization, but still need to heal from an injury or illness (including behavioral health conditions) and whose condition would be exacerbated by an unstable living environment

Respite Services are provided to caregivers of Members who require intermittent temporary supervision. The services are provided on a short-term basis because of the absence or need for relief of those persons who normally care for and/or supervise them and are non-medical in nature

Day Habilitation Programs are provided in a Member’s home or an out-of-home, non-facility setting. The programs are designed to assist the Member in acquiring, retaining, and improving self-help, socialization, and adaptive skills necessary to reside successfully in the person’s natural environment

Nursing Facility Transition/Diversion services empower individuals to maintain community-based living and prevent unnecessary institutionalization by providing comprehensive support for those requiring skilled nursing care. These services offer flexible alternatives to long-term nursing facility placement, enabling eligible members to choose community-based settings like assisted living facilities, with the primary objectives of facilitating safe transitions from institutional care and supporting individuals in maintaining their independence and quality of life.

Community Transition Services/Nursing Facility Transition to a Home provides comprehensive support for individuals moving from licensed facilities to independent living arrangements, focusing on helping members establish self-sufficient households. These services encompass a holistic approach that includes assessing housing needs, securing appropriate housing, completing necessary documentation, coordinating with landlords, arranging transportation, identifying environmental modifications, and establishing procedures to ensure successful long-term housing stability for individuals transitioning from institutional settings to community-based living.

Personal Care Services and Homemaker Services provide essential support for individuals requiring assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), enabling them to maintain independence and remain in their homes. These comprehensive services encompass a wide range of support, including personal hygiene, mobility assistance, meal preparation, household management, medical appointment accompaniment, and protective supervision for those with mental impairments. By addressing both basic and complex daily living needs, these programs help vulnerable individuals maintain their quality of life and avoid institutionalization.

Environmental Accessibility Adaptations (EAAs also known as Home Modifications) are physical adaptations to a home that are necessary to ensure the health, welfare, and safety of the individual, or enable the individual to function with greater independence inthe home: without which the Member would require institutionalization.

Medically Tailored Meals provide targeted nutritional support for individuals with chronic conditions, delivering personalized meals designed by registered dietitians to address specific medical needs and dietary restrictions. By focusing on individual health requirements, these services aim to reduce hospital readmissions, improve nutritional health status, and enhance overall member well-being through comprehensive, evidence-based dietary interventions.

Sobering Centers provide a compassionate, alternative intervention for individuals publicly intoxicated, primarily targeting homeless or unstable populations. These centers offer a safe, supportive environment with comprehensive services including medical triage, temporary shelter, basic healthcare, substance use counseling, and navigation to additional health and social services. By coordinating with behavioral health agencies, law enforcement, and using trauma-informed, harm reduction approaches, Sobering Centers aim to provide immediate care and support pathways for individuals experiencing complex health and substance use challenges.

Environmental Asthma Trigger Remediations are physical modifications to a home environment that are necessary to ensure the health, welfare, and safety of the individual, or enable the individual to function in the home and without which acute asthma episodes could result in the need for emergency services and hospitalization
